What information do I need to provide when signing up for NDIS support?

You will generally need to provide your name, date of birth, contact details, address, NDIS number, NDIS plan dates and funding type. You may also need to provide invoicing details, representative information and an emergency contact.

Yes. The Participant Sign Up Form allows you to identify whether your NDIS funding is NDIS-managed, self-managed or plan-managed. If you are NDIS-managed, a copy of your NDIS plan must be provided as part of the sign-up process.

Yes. The form allows a participant to provide representative details, including a parent, guardian, caregiver or another authorised representative. The person signing the agreement must also provide their name and relationship to the participant where applicable.

What happens after I submit the Love Home Nursing participant form?

After you provide your participant information and complete the required authorisations, Love Home Nursing can use the information to understand your support requirements and NDIS funding arrangements.
